Part Time Wellness Educator
Hackensack, NJ
This is a part-time temporary 12 month assignment (This position is grant-funded and is contingent upon the continued availability of grant funding) up to 20 hrs. a week.
Who is Bergen's Promise?
Bergen's Promise is the Care Management Organization (CMO) of Bergen County as part of the New Jersey Children's System of Care. We are a rapidly growing, mission driven, non-profit social service agency. We are accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF).
Bergen's Promise is a 501(c)(3) non-profit organization dedicated to meeting the complex mental health and wellness needs of children up to age 21 with identified emotional and behavioral challenges, substance use issues, and intellectual and developmental disabilities.
Job Responsibilities
- Engage youth and families in health promotion activities to support healthy lifestyle changes through motivational interviewing.
- Provide health education specific to SAMHSA’s 8 Dimensions of Wellness.
- Participate in the development of SMART self-management health goals.
- Facilitate regular in-home meetings with youth and families to monitor progress.
- Adhere to all Medicaid regulations, HIPAA standards, and agency documentation requirements.
Requirements
To be considered for this role, you must possess:
Qualifications/Skill Requirements:
· Minimum Bachelor’s Degree in public health, health education OR related field
· CHES® certified, or eligible to sit for examination
· Bi-lingual language skills preferred
· Maintain a high level of teamwork, leadership, and professionalism
· Culturally competent and responsive
· Excellent public speaking and interpersonal communication skills
· Ability to work with energy, creativity, and enthusiasm on assigned teams
· Valid driver’s license is required; MUST maintain required vehicle insurance
