Salary: $73,375- $95,000

Position Summary: Provides direct, consultative, and supportive assessment and therapeutic services in psychotherapy and cognitive rehabilitation therapy to persons served and outpatients to enhance individual potential for learning and to meet the specific rehabilitation needs of persons served.
Responsibilities
- Provide clinical social work, case management, and care coordination services in compliance with applicable federal, state, accreditation, and organizational standards.
- Conduct comprehensive psychosocial assessments and develop individualized treatment, transition, and discharge plans.
- Provide individual, group, and family therapy, as appropriate, using evidence-based practices.
- Serve as the primary liaison between individuals served, families, schools, residential programs, community agencies, funding sources, and interdisciplinary team members.
- Coordinate and facilitate Individual Service Plan (ISP), Individual Education Plan (IEP), and Interdisciplinary Team (IDT) meetings to monitor progress and update treatment goals.
- Collaborate with clinical, educational, residential, and community partners to ensure continuity of care and successful transitions.
- Advocate for individuals served by promoting person-centered care, self-determination, dignity, and access to appropriate services and resources.
- Maintain timely, accurate clinical documentation, treatment plans, assessments, and regulatory reports in accordance with organizational and licensing requirements.
- Communicate regularly with families, guardians, case managers, and community providers regarding treatment progress and discharge planning.
- Participate in admissions, discharge planning, case conferences, quality improvement initiatives, and interdisciplinary meetings.
- Respond to crises and emergencies using approved intervention techniques while maintaining a safe therapeutic environment.
- Maintain current licensure, required certifications, and continuing education while adhering to ethical standards, confidentiality requirements (HIPAA), and organizational policies.
- Perform other related duties as assigned to support program operations and quality care.
Qualifications
Master degree in Social Work with a minimum of 3-5 years of related work experience preferred. Current N.J. Licensure (NJLSW) required and must work to obtain NJ Licensed Clinical Social Worker Licensure (NJLCSW). Current NJ Licensed Clinical Social Worker (NJLCSW) preferred. Prior experience working with developmentally-disabled individuals preferred.
