We’re seeking a compassionate and dependable Licensed Practical Nurse (LPN) to serve as a Group Home Nurse, overseeing the health and medical needs of individuals living in a community-based residential setting.
This is an opportunity to use your nursing skills outside of a traditional healthcare environment while developing meaningful relationships with the individuals you support. You’ll provide hands-on nursing care, coordinate medical services, manage medications, and work closely with physicians, caregivers, and program staff to promote each resident’s health, safety, independence, and quality of life.

What You’ll Do
As the Group Home LPN, you will provide clinical oversight, coordination, and direct nursing intervention related to the health and medical needs of individuals living in the home.
You’ll work closely with residents, caregivers, program staff, physicians, and other healthcare professionals to ensure medical needs are identified, appropriately addressed, documented, and incorporated into each individual’s overall support.
Your Responsibilities May Include:
Nursing Care & Health Monitoring
- Provide hands-on nursing and clinical care to individuals living in the home.
- Assess residents’ health needs and respond appropriately to medical concerns.
- Monitor ongoing health conditions and identify changes requiring additional attention.
- Provide direct nursing interventions based on individual health needs.
- Promote preventive healthcare, wellness, and healthy living.
- Monitor residents following medical treatments, illnesses, or changes in condition.
- Communicate significant health concerns to appropriate program leadership and healthcare providers.
Medication Management
- Administer medications and treatments according to physician orders.
- Oversee medication management and medication safety.
- Maintain accurate medication documentation and records.
- Monitor individuals for medication effectiveness, side effects, or other concerns.
- Communicate medication-related concerns to physicians and other appropriate healthcare professionals.
- Help ensure medications and treatments are provided according to required procedures.
Medical Appointments & Care Coordination
- Coordinate medical appointments and healthcare services for residents.
- Communicate with physicians, specialists, pharmacies, caregivers, and other healthcare providers.
- Follow up on medical recommendations, appointments, and prescribed treatments.
- Help ensure continuity of care between medical providers and residential staff.
- Maintain current health information and communicate appropriate updates to the care team.
- Advocate for residents and help ensure their individual healthcare needs are addressed.
Staff Training & Support
- Provide health-related guidance and support to residential staff.
- Train staff on individual health needs and appropriate health-related procedures.
- Educate staff regarding medications, treatments, health conditions, and signs or symptoms requiring additional attention.
- Reinforce safe practices and established medical protocols.
- Serve as a nursing resource for program staff when health-related questions or concerns arise.
- Collaborate with caregivers and program staff to promote consistent, high-quality support.
Documentation & Compliance
- Maintain accurate and timely nursing documentation.
- Maintain medical records and other required health documentation.
- Protect confidential medical and personal information.
- Help ensure nursing practices comply with applicable state regulations, agency policies, and medical protocols.
- Follow HIPAA requirements and maintain confidentiality.
- Follow applicable OSHA, fire, safety, and organizational policies.
- Attend required meetings and trainings.
- Maintain a professional demeanor and exercise sound clinical judgment.

Qualifications
Required
- Current and valid Licensed Practical Nurse (LPN) license
- High School Diploma or equivalent
- Strong clinical knowledge and nursing practice skills
- Medication management and safety awareness
- Strong assessment and critical-thinking skills
- Ability to maintain accurate medical documentation
- Effective communication and interpersonal skills
- Ability to respond appropriately to urgent medical or health-related situations
- Ability to maintain confidentiality and follow ethical nursing practices
Additional Requirements
- Current Driver’s License
- Current vehicle registration
- Access to a functional vehicle
- Basic computer skills and knowledge of standard office applications
- Ability to complete all required training
- Ability to work collaboratively with residential, clinical, and program staff
Schedule & Compensation
- Full-Time
- $28.00/hour
- Community-based residential/group home setting
- Schedule flexibility may be required based on resident and program needs.
- Flexibility may include occasional evenings, weekends, or work at different program locations.
Physical Requirements
- Ability to sit, stand, reach, bend, stoop, twist, and climb steps.
- Ability to drive a vehicle as required.
- Ability to lift 50 pounds or more as required by the position.
- Ability to work in residential, office, and community-based environments.
- Ability to perform essential job functions with or without reasonable accommodation.
