Job Description
E & S Home Care Solutions is seeking a compassionate, reliable, and experienced Registered Nurse (RN) Field Nurse to join our growing home care team.
We are looking for a dedicated RN who enjoys working independently, conducting in-home assessments, and ensuring that clients receive quality care in the comfort of their homes.

Requirements
Key Responsibilities
Conduct initial nursing assessments and reassessments in clients' homes.
Develop, review, and update individualized plans of care.
Perform supervisory visits for Home Health Aides (HHAs).
Evaluate clients' health conditions and identify changes in care needs.
Ensure compliance with New Jersey home care regulations and agency policies.
Complete all required nursing documentation accurately and within established deadlines.
Communicate effectively with clients, families, caregivers, and office staff.
Provide guidance and education to HHAs regarding client care needs.
Qualifications
Active and unrestricted Registered Nurse (RN) license in New Jersey.
Previous home care or home health experience preferred.
Strong nursing assessment and documentation skills.
Excellent communication and organizational skills.
Ability to work independently and manage field visits efficiently.
Valid driver's license and reliable transportation.
