Job Overview
The Nurse Home Visitor provides in-home support and education to first-time mothers, guiding them through pregnancy and the first two years of their child’s life. They focus on improving maternal and child health outcomes, supporting family economic self-sufficiency, and connecting clients to community resources as needed. Through building long-term therapeutic relationships, conducting nursing assessments, and collaborating with clients to set goals, they deliver care in alignment with the Nurse-Family Partnership model.
As an integrated health organization Acenda values the use of evidence-based practice or EBP's.

The Nurse-Family Partnership Program is a free service that pairs a first-time mom by 28 weeks gestation with a personal registered nurse to ensure the best possible pregnancy outcome, the best health and development of the baby, and to help link mom to community resources.
