DescriptionThe nurse navigator helps patients “navigate” through the often-complicated journey of a cancer diagnosis. Throughout this process, the nurse navigator is there to help resolve any barriers to timely care, help connect patients with the resources they may need, and to provide education and emotional support throughout the phases of diagnosis, staging, active treatment and survivorship.
Principal Accountabilities:
- Provides a comprehensive assessment of the oncology patient, family and / or caregivers needs at pivot medical visits across the continuum, including assessing barriers to care and distress screening, refer to appropriate disciplines.
- Provides education related to oncology diagnosis, treatment, cancer center support services and community resources.
- Facilitates timely coordination of appointments, tests and procedures communicating any concerns to the appropriate physician/s. develop assessment tools, guidelines, pathways and algorithms across the continuum of care.
- Educates the patient, family and/or caregiver to clinical trials, coordinate with Cancer Research Department
- Participates in the multidisciplinary tumor conference and document recommendations as outlined by Commission on Cancer Standards. To monitor recommendations for appropriateness with NCCN or other national guidelines.
- To participate in performance improvement activities to enhance the patient, family and / or caregiver experience across the continuum.
- To develop in conjunction with support staff support groups and education programs for the patient, their families and/ or caregivers.
QualificationsRequired:
- RN license and maintain required credentials
- BSN
Preferred:
- Oncology nursing certification preferred or certification completed within 2 years.
- Three or more years of experience in oncology a plus
- Requires problem solving decision making and critical skills thinking.
